As a  legal adult, I release and hold harmless Elite Dance Academy, its owners, administrators and instructors from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by while in or upon the premises or any premises under the control and supervision of Elite Dance Academy, its owners, administrators or in route to or from said premises from and forth. Elite Dance Academy is hereby granted permission to take photographs and video of me to use for promotional purposes. Permission is also hereby granted to Elite Dance Academy to copyright such photographs and video in its name.

I also agree to the following guidelines when visiting Elite Dance Academy:
I confirm I do not have any symptoms related to and have not been knowingly exposed to COVID19 in the last 14 days
I understand that EDA is following the State Mandates for mask wearing.
I agree to social distancing while at the studio
I agree to keep my hands washed and sanitized while at the studio
I agree to not bring food into the studio
I agree to limit the amount of belongings I bring and keep them in the designated area at all times
 
I HAVE READ AND UNDERSTAND ALL OF THE ABOVE INFORMATION, AND I AM WILLING TO COMPLY WITH THE ABOVE STATEMENT SET FORTH BY ELITE DANCE ACADEMY, ITS OWNER, DIRECTORS, AND EMPLOYEES.
