In accordance with the Family Educational Rights and Privacy Act of 1974, 20 U.S.C. § 1232g (“FERPA”), the undersigned student hereby permits his or her college/university to disclose the information specified below to Black History University:
Financial Aid Records (e.g., status of file, award and disbursement of funds information, Satisfactory Academic Progress status, income information, and any other relevant information contained in the student’s application for financial aid or financial aid file)
Academic/Transcript Records (e.g., transcripts, application for admission, admission letter/package and registration information).
Student Account Records (e.g., amount for cost of attendance, sources of payment for cost of attendance, refund information and any other accounts receivable information contained in student account records).
