 ***Agencies interested in joining Minnesota's HMIS are temporarily unable to do so, owing to the ongoing HMIS software transition from WellSky Community Services to Eccovia ClientTrack. The go-live date for the new HMIS is July 1, 2024, after which agencies will once more be able to join the system. The New Agency Inquiry Form remains open for submissions, and agencies interested in HMIS participation are encouraged to fill it out so that ICA can review requests and follow up with questions as needed. Please note that responses to form submissions may be slower than usual as a result of software transition-related work being prioritized.***

This form should be completed by agencies that are interested in participating in Minnesota's HMIS. The information collected here will provide ICA with a baseline understanding of your agency and how you intend to use HMIS. After form submission, ICA staff will either follow up with next steps for getting your agency set up in HMIS or request further information to determine if HMIS is the right system for your data entry and reporting needs.
