TERMS AND CONDITIONS and INDEMNITY FORM 1. As parent and/or legal guardian of the above mentioned player, over whom I have custody and control, I hereby consent to them participating in the various activities of the coaching clinic / indoor hockey arranged, organised or offered by Pitbulls and Pro Series Indoor (PSI). 2. As parent and/or legal guardian of the above mentioned player I will hold blameless and indemnify all persons, Pitbulls, PSI and all other organisations associated with the activity (including the host venue), should any prejudice, loss, damage, illness or injury occur to the above mentioned player arising from any reason whatsoever during the period of the activity. This includes an indemnity against recovery of costs resulting from damage, loss and/or medical conditions or hospitalisation.3. As parent and/or legal guardian of the above mentioned player I appoint the Pitbulls and PSi Coaches accompanying the group, or supervising the activity, to act in loco parentis in respect of the afore mentioned player should the need therefore arise. 5. Pitbulls and PSI reserves the right to cancel the course & refund your money should not enough players register. 5. No pay no play policy. First come first serve basis (this means valid registration AND payment in full). 6. No refund policy should the event go ahead. 7. The right of registration and attendance is reserved. 8. Upon checking the “Accept” box during the registration process, the parent and/or legal guardian has indicated that they have read and accepted the terms and conditions and indemnity above. *
