Please complete the form below to request applications for each of OLSH's sponsored scholarships.

To view descriptions for each of these scholarships to determine which ones you would like to apply for, please go to our website at www.olsh.org/apps/pages/scholarships.

Within one week of request, applications will be individually shared with you via link to OLSH's Blackbaud System, where the applications will be shown as online forms for completion. All completed scholarship applications are due no later than March 15, 2023.

9th Grade Merit-Based Scholarships *
Please note: The St. John Neumann Endowed Scholarship and the OLSH Merit Scholarship require no application. Students who take the OLSH Placement Test are automatically eligible for these scholarships.

These scholarships require that you apply and qualify for OLSH financial aid through FACTS.

Please note: The Collier Family Incentive Scholarship(s) are awarded based upon the applicants to the Poleski Family Endowed Scholarship. It requires no additional application. Likewise, the Mary E. Falbo Petrucci Memorial Scholarship requires no application.
