Please complete this form if your child(ren) are interested in participating in the 2024-2025 Special Olympics Team Events. 

Specific dates TBA, but those months are usually: 
Basketball (October annually)
Bowling (February annually)
Track & Field (April annually)

Please be looking for paperwork in the upcoming months for official registration from your child's teacher.
Special Olympic Medical Release Form and current physical paperwork must be on file for your child to participate.
