MINI Soccer Camp for Christ (SK-Gr 2) Registration 2022

AFTER forms are submitted and we email you that a spot is available, registration must be confirmed by the $40 fee, through e-transfer to treasurer@chathamgrace.com, password: Soccer2022 or cash/cheque brought to Grace CRC. This will be non-refundable, and covers costs of a soccer ball and t-shirt. If not completed within one week, you will be moved to the end of the waiting list.
Children must be entering SK-grade 2 in September to be eligible for Mini Soccer Camp 4 Christ 2022.

Behaviour Expectation Policy: If a child is struggling to participate well in his or her team at soccer camp, his or her team leader will ask one of our designated Head Coaches to assist the child until the issue can be resolved.  If the issue cannot be resolved after 15 minutes of one-to-one assistance with a Head Coach, the Camp Director will contact the child’s parent/guardian and ask that the child be picked up from camp as soon as possible.  The child may attend camp the next day. Payment Information: We will again be accepting e-transfers to cover the registration fee. AFTER we have confirmed that there is a spot available for you, please send your e-transfer to treasurer@chathamgrace.com and use the password: Soccer2022  If you cannot use e-transfer, please comment below and bring payment, clearly labelled, to Grace CRC during office hours or to the lockbox by the northeast doors.
On Thursday, August 18, at 5:00pm, there will be a free BBQ at Grace CRC.  Friends and family are very welcome and we look forward to meeting you! Please note that children will require your supervision. Please indicate how many persons will be attending the BBQ (including the child registering for camp).  If you are filling out registration forms for more than one child, please include all guests on just one form.
Protecting your Personal Information: Your child's health and personal information is collected to ensure the safety and well-being of each person involved in our camp ministry.  This information will only be seen by our camp volunteers and will be kept in a secure place.  I have read and understood this explanation. *
Medical/Video/Photography Authorization: I hereby authorize the staff and volunteers of the Soccer Camp 4 Christ to make any and all decisions regarding the emergency treatment of my child.  I also hereby authorize the staff of the Soccer Camp 4 Christ to take video and still photos of my child during camp.  I have read, understood and agree with the above and hereby release and discharge all parties associated with this camp from any and all claims, demands, actions, and causes of action, that I/we or my/our child(ren) incur(s).   *
