After filling out this application for Light Youth Winter Retreat 2024, you are on the list, but to secure your spot at camp you need to PAY $200 (per person). All payments are to be made either through PushPay (choose Youth Camp option) or in person to Masha Kirilko or Mark Bondarchuk.

**LIABILITY**
Light to the World Church and its staff and volunteers acting on behalf are hereby released from any and all liability in the event of any accident or misfortune, damage or loss that may occur to you and your property unless it is due to our negligence. You hereby give permission to the First Aid staff/paramedics to ensure proper treatment. You agree to pay all such doctor, ambulance and hospital fees incurred to you. You have attached a list of any health information about yourself that the First Aid staff/paramedics need to know.

**CODE OF CONDUCT**
You agree to have positive and appropriate manner with your fellow campers, staff, and other members at camp. Negative behavior (including, but not limited to: harassment, verbal threats, physical behaviour, bullying, etc.) that impacts others physically or emotionally will result in immediate dismissal or removal from camp at the discretion of church, and camp administrators. You will be automatically dismissed from camp for the use of illegal drugs, alcohol, cigarettes, and weapons, or for engaging in theft, vandalism, or violent behavior. Campers may also be removed from camp as a result of behavior that places them at risk or whose conduct is disruptive or otherwise deemed unacceptable. Refunds will not be granted for dismissal prior to the end of the camp day or week.
