Before completing this form you will need to have your players 2021-2022 USA Hockey registration number and have completed a fitting (with chest, elbow and shin pads) at Westwood Sports in Apple Valley. You can obtain your USA Hockey Number by visiting their website at https://membership.usahockey.com/.

By completing this order form you are asking the Apple Valley Hockey Association/Burnsville Hockey Club Executive Boards to purchase game socks and jerseys your players behalf. You are also agreeing to reimburse the clubs $150 for the customized jerseys if your player opts out of playing for the Firehawks during the 2021-2022 season.

Orders submitted after July 21 will be assessed a $50 late charge and jerseys may not arrive in time for the start of the season.

Please identify five preferred jersey numbers (1-99) in order of preference. Seperate numbers with a dash. (ex. 2-22-27-2-82) *

NOTE: In situations where two players in adjacent grade levels request the same first choice, the older player will have seniority. For example, if a 5, 6th and 7th grader all request number 13 as their first choice, the 5th and 7th grader would be assigned number 13 since the 7th grader has seniority. The 5th grader could also have number 13 because a 7th and 5th grade player would not be playing regular season games at the same level.
