Use this form if you are a MAT booking two or more schools onto the same DfE approved and fully funded Senior Mental Health Lead two-day training course. The course is followed by 6 months of support as the staff carry out their role and 12 months individual Teach Well Toolkit membership. You will also be sent a free copy of Steve Waters' book, 'Cultures of Staff Wellbeing and Mental Health in Schools' Open University Press/McGraw-Hill which we will refer to during the training.
By booking two or more schools in your MAT onto the same course, you will receive up to 20% discount, depending on the number of schools you register.

Please note: All state schools in England are entitled to free Senior Mental Health Lead training. Funding is guaranteed until 2025. The cost of the training for an individual school is £895 and each school receives £1200. You can use the surplus on any aspect of wellbeing and mental health that you wish. If you are an independent school outside the state school system, you are not eligible for funding. The cost of the training is £795.
* By booking two schools onto the same course, the cost of the course is discounted by 10% to £805.50, saving each school £80.55.*
*By booking three schools onto the same course, the cost of the course is discounted by 15% to £760.75, saving each school £134.25.*
*By booking four or more schools onto the same course, the cost of the course is discounted by 20% to £716 £760.75, saving each school £179.*
This form is to book each of your schools a place on a Teach Well Toolkit Senior Mental Health Lead course only. We do not process your funding application. You do this directly with the DfE, after we provisionally confirm your place via email on receipt of this form.
When applying for funding, you use our course code SMHL093 to enable the DfE to identify Teach Well Toolkit on their database of approved providers. WE WILL SEND YOU AND YOUR FINANCE OFFICER A GUIDE TO APPLYING FOR FUNDING, ATTACHED TO YOUR PROVISIONAL CONFIRMATION.
Your finance officer will be sent an invoice. You will be copied in to this email. After we have received payment from your school, either within 90 days or before the training, whichever is sooner, you will receive your copy of Steve Waters' book and, approximately a week before the training course, instructions on how to join the training.
SUMMARY OF STEPS:
1) Complete this booking form to apply for places on Teach Well Toolkit's Senior Mental Health Lead course.
2) We will provisionally allocate your places and notify you each member of staff of this via email. A guide to applying to the DfE for funding will be attached to our email.
3) Each school applies to the DfE for funding, quoting our course code SMHL093.
4) We will send you either one invoice or separate invoices to each school, depending on which you have requested on the form below.
5) When each school has received their funding from the DfE, the MAT either pays us centrally for the number of places on the course or each school pays us separately, depending on which you have requested on the form below.
6) We confirm your place on the Senior Mental Health Lead course.
7) We will send you a copy of Steve's book: 'Cultures of Staff Wellbeing and Mental Health in Schools'.
7) Approximately one week before the course, we will send you joining instructions.
