Please note that this is NOT an official registration or guarantee that we will be able to hold in-person classes this fall. However, ONLY families who have completed this form will be contacted to schedule any in-person classes.

By completing this form, you agree to our new in-person class policy. The policy is subject to change based on state guidelines, but at the moment will include: required wearing of masks for adults (and possibly older children), providing one's own seating (if outside) and instruments for class, and not having anyone in the household showcase COVID symptoms (i.e. Fever or chills, cough, shortness of breath or difficulty breathing, fatigue, muscle/body aches, headache, new loss of taste/smell, sore throat, congestion or runny nose, nausea or vomiting, diarrhea) within 24 hours of attending a class. *
