We can't wait to Spring into Dance with you! We will be abiding by all necessary safety measures to keep attendees safe, since COVID-19 still presents challenges. Attendees are required to act in accordance with local law and as a condition of entry, you are required to READ AND SIGN (by typing in your name) this release form, which indicates your compliance with event precautions.

Glen Rock TV will be filming and photographing the event. *

I am aware that GRTV will be filming and photographing. I hereby grant the Arts Council of Glen Rock permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications, without payment or other consideration.

In attending our event, you will be a guest of the Thielke Arboretum and will follow all outlined thielkearboretum.org/rules--regulations . *

I will respect and honor the environmental mission of the arboretum while visiting and will abide by its rules and regulations.

The event requires masks at all times and encourages participants to remain socially distant whenever possible. *

I will wear a mask at all times and follow all other Covid-related precautions.

Spring Into Dance: Glen Rock, the Arts Council of Glen Rock, Friends of the Glen Rock Arboretum, Inc., and the Borough of Glen Rock are not legally responsible for any injury or illness, however unlikely, that occurs during the event. *

I am medically able to participate and aware that risk of injury is inherent in any physical activity. I assume the risk of any such injury.

I agree to indemnify and hold harmless the Arts Council of Glen Rock, Friends of the Glen Rock Arboretum, Inc., the Spring into Dance: Glen Rock event, and the Borough of Glen Rock, their Officers, Agents, Volunteers and Employees from any and all liability, claims, damages, court costs, and attorney fees arising from any use of the Thielke Arboretum, including, but not limited to Volunteering, Attending Events, Use of the Grounds and Property and use of the Vielbig/Scerbo EEC.

I understand that this Hold Harmless Agreement also requires that the Arts Council of Glen Rock, Friends of the Glen Rock Arboretum, Inc., the Spring into Dance: Glen Rock event, and the Borough of Glen Rock are indemnified from any losses or damages resulting from the acts or omissions from any guest, participant, visitor, or other person using Thielke Arboretum as described above.

By typing your name below, you are signifying that you have read and agree to the requirements stated in this release. All attendees are required to sign this release as a condition of entry. A parent or guardian must sign on behalf of all minors. *
