2023 - 24 Change of Member Status Form
This Change of Member Status Form should be completed every time a new (uninitiated) OR initiated member's status changes after the beginning of the semester chapter roster audit/review and before the end of the semester chapter roster audit/review.  Please complete this form for:

1) Students who have selected to terminate their collegiate fraternal affiliation with the chapter (i.e. remove them from the roster), or

2) Students who have been removed from associate or active membership by the chapter (i.e. remove them from the roster), or

3) Students who have joined the organization through a Continuous Open Bidding Process or Informal Recruitment (i.e. add them to the roster).
