All Skutt Catholic students will receive the outstanding 2022-2023 SkyHawk Yearbook and have the cost of $85 added to their FACTS account February 28th with funds being pulled on March 10, unless a parent/guardian completes this form to opt out of purchasing a 2022-23 Yearbook. If you want your child/children who are students at Skutt Catholic to get a yearbook you do not need to do anything or complete this form. Only complete this form if you DO NOT want a yearbook for your child. If you have more than 1 child at Skutt Catholic who you do not want to get the yearbook, then fill out a separate form for each child.
