This Intermediate Clinic is for children in grades 4th - 12th. Payment must be received by April 7th or the spot will be released to another child. Should you have any problems registering please contact us at the office. The phone number and email address is noted at the top of the form.
 You can mail a check to: Foley Parks and Recreation/Tennis Clinic   Attn: Keisha Ellis     P.O. Box 1750     Foley, AL 36536  or stop by our office at 121 N. Alston Street to pay the registration fee. If you are mailing a check please make the check payable to "City of Foley" and include your phone number and drivers license number on the top of the check. Also, please include your child's first and last name in the memo section of the check. All registration fees must be paid by April 7th.
A copy of your responses will be emailed to the address you provided.
