Transcript Request Form (Current Foran High Students Only)
Use this form to request transcripts for scholarships, summer programs, athletic recruitment, DMV, etc.  
Please note: 
  • OFFICIAL TRANSCRIPTS are signed and sealed in an envelope.  They are sent directly from Foran to scholarships, programs, recruiters, institutes or can be picked up at Foran High School by the student requesting it.
  1. DMV official transcript requests need to be picked up.
  2. SCHOLARSHIP official transcript requests - The completed application needs to be given to Ms. Collins one week prior to the deadline date to be sent out.
  • UNOFFICIAL TRANSCRIPTS can be emailed to the student.  Please state the reason you need the transcript.
Please be specific as to what you need.
Also, allow at least 3-5 school days to fulfill your request.
