The Summer Program (SSP) 2024              Pre-Enrollment Form
The 2024 Summer Program will be held at Hellstern Middle School in Springdale and Folsom Elementary School in Farmington, for grades K - 6th, from June 3 through August 2, 2024. Your child's grade will be determined by the grade they are entering for the 2024/25 school year. The hours of operation are 7a - 6p, Monday thru Friday.

SSP Rates
$75 registration fee per child
1 child: $180/week tuition
2 children: $350/week tuition
3 children: $500/week tuition
4 children: $600/week tuition
I understand that the 2024 Summer Program is only accepting full time enrollment, and that I will be responsible for the full rate of tuition from June 3 - August 2, 2024. *
Please list any special schedule requests. If full-time enrollment fills up and availability remains, special schedules will be considered.  *
I agree to the following statement:
I understand that completion of this pre-enrollment form does NOT guarantee my child's(children's) enrollment in the Summer Program. Pre-Enrollment forms for summer will be accepted through March 15, 2024. I will be notified by March 29, 2024, if my child(children) is(are) accepted.
