Risk Assessment and Acknowledgement

Required to be completed by spectators to athletic events prior to entering SAU #39, Amherst School District, Mont Vernon School District, and/or Souhegan Cooperative School District Facilities.

This questionnaire must be completed on each day you will be accessing a facility.

It is the intent of SAU #39 and its participating school districts to try to limit the potential risk to employees and others entering district facilities of being exposed to the COVID-19 virus in school facilities.  Individuals requesting access to a facility are required to answer the following questions to determine the appropriateness of onsite work.  The questions were developed utilizing information provided by the Centers for Disease Control.  If you answer "yes" to any of the questions below, please contact Amina Fazlic, Director of Human Resources @  afazlic@sau39.org or Denise Raymond, school nurse @ draymond@sprise.com, to discuss your particular circumstance prior to entering a school facility or attending a school function.
