23-24 School Year Application and Parent Agreement Form for ABLE (Academy of Blended Learning Education)
If you have a student behind in multiple credits, and are interested in our credit recovery program, please complete the following form and questionnaire.  This application lists all the non-negotiables that all students must agree to along with the ways parents/guardians can support their student's success in the academy. Read carefully as you fill it out, and we will get in touch with you as soon as we can to set up an interview.

This should be filled out only by a parent/guardian (unless the student is 18 and on their own).  After the application is submitted, the ABLE counselor will contact you and set up an interview where the parent/guardian and student must be present to go over and sign the student/parent/teacher contract.
