Please note there is a Medical Release Form that is required and must be notarized. Please return all notarized forms along with a $75 deposit to hold your child(ren)'s spot. MEdical forms and deposits can be dropped off at the Lawrence Ave Main Office  or mailed to Latch Key, 29 Leroy Street, Potsdam, NY 13676.
