This is the OFFICIAL ORDER FORM for our t-shirts. Please fill out if you are ordering a SHORT-SLEEVED t-shirt, as we will be ordering one for you based on this list. The cost of red shirt with the name on the shirt will be $15.00, and the cost of regular(NO NAME) will be $13.00. This is a COMMITMENT TO PURCHASE AND YOUR STUDENT ACCOUNT WILL BE CHARGED IF YOU DO NOT PAY THE REQUIRED PAYMENT.
