FL District Kids Camp 2022 Registration

1. Fill out this form in its entirety -  (Please do not fill out the form in all CAPS)

2. Print and fill out the Information Sheet Notarized Medical Form (included at the end)

3. Give the Notarized Medical Form and Deposit to your church's camp coordinator prior to the due date (June 5th)


                     Steps 1 - 3 must ALL be completed by the due date to secure the desired price.

Please give all forms and money to your church's camp coordinator and they will mail them in. Please do NOT mail them to the District Office or the campground. Thank you!
