Health and Safety:

I am aware of the rules and regulations which govern participation in any White Rose Cheer Activities (including but not limited to all aspects of cheerleading, tumbling, and dance training) run by White Rose Cheer and understand that it is my duty to keep up to date with the changes in policies and procedures.
I am fully aware that any activity involving motion, height or other athletic activity creates the possibility of serious injury. I accept that participation in these activities with White Rose Cheer is at my risk, including my responsibility for any minor that I am acting on behalf of as either the parent or legal guardian. I confirm that I will indemnify and hold harmless White Rose Cheer, its coaches and any associated business from liabilities and claims subject to applicable law.
I confirm that I have completed the information required by White Rose Cheer on Coacha to the best of my knowledge and understand that any failure to disclose information requested may result in possible injury which would be unknown because of such failure and I will, therefore, indemnify and hold harmless White Rose Cheer, its coaches and any associated business from any liabilities or claims because of my failure to disclose information.
I confirm that I will immediately update the disclosed information as needed because of any changes in circumstances or medical conditions and that I will indemnify and hold harmless White Rose Cheer, its coaches and any associated business from liabilities and claims if I fail to do so, including emergency contact details and home address.
I accept that White Rose Cheer can, at its sole discretion, refuse to allow participation by the named participant on the basis of medical concerns.
I confirm that if either I or the participant for whom I am acting as parent/legal guardian is taken ill or injured during any activities run by White Rose Cheer either in training sessions, events or competition then I consent to initial treatment being administered by a trained first aider. If the emergency contact cannot be contacted, I consent to further urgent treatment being administered by either paramedics or a doctor. I also confirm that I will accept all legal and financial obligations incurred because of such action.

Consent for filming/photography:

The taking of photographs and/or video recordings is very much a part of White Rose Cheer activities but we are aware that this may be incompatible with a participant’s situation because of child protection or safeguarding issues. If this is the situation with the participant then please discuss with us on a confidential basis so that we can make appropriate accommodation which may limit the participant’s participation in events and activities.

I hereby consent to White Rose Cheer taking and using images of the participant (either myself or the minor for whom I am the parent/legal guardian) taken either as photographs or as a video recording.
I also consent to such photographs or video recordings being used for marketing and publicity related purposes and that they may be published via both printed and social media or websites of both White Rose Cheer or other websites associated with White Rose Cheer.
I understand that when taking part in competitions or public performances this permission is required so that the event organisers may use photographs and/or video recordings for their own publicity and I accept that such usage is outside the control of White Rose Cheer. I therefore accept that registering as a team participant will automatically give consent to White Rose Cheer and event organisers for the use of such photographs and/or video recordings.
White Rose Cheer will hold images in accordance with the provisions of GDPR.
White Rose Cheer will never use full names when publishing any images.
I hereby agree that the images of the participant captured by White Rose Cheer in photographs and/or video recordings will be the copyright of White Rose Cheer and that any other intellectual property rights which arise in the recordings will also be the property of White Rose Cheer.
I and any minor for whom I am the parent or legal guardian, hereby agree to irrevocably assign all intellectual property rights in the participant’s performance and/or recordings to White Rose Cheer.
I and any minor for whom I am the parent or legal guardian, hereby agree to waive all moral rights in the participant’s performance and/or recordings to White Rose Cheer.
I understand that I can ask White Rose Cheer to stop using the participant’s photographs and/or any video recordings containing the participant at any time by making such a request in writing. In this situation, such photographs and video recordings will not be used in any future publications but may continue to appear in publications already in circulation.

COVID-19:

I hereby agree to fully and completely comply with all social distancing, hygiene and other preventative measures relating to Covid-19 and its spread. I have read all documentation, particularly the "COVID-19 WRC Phases C-G" document, available to me through the White Rose Cheer website/Facebook Page/Email Documentation, understand the contents and have taken opportunities to have my questions answered. I hereby agree to fully inform Program Director Kelly Loughlin via email on whiterosecheer@gmail.com if my athlete or any member of my household is self-isolating, or has been diagnosed with Covid-19. I hereby agree to not attend cheerleading if my athlete has been told to self-isolate via a third party (i.e. school or place of work).

I understand that if me/my athlete presents at the gym with symptoms (persistent cough, high temperature above 37.7, or loss of taste or smell) that they will be required to be collected immediately.

I understand that I am expected to order a test by calling 119, and agree to inform White Rose Cheer of the result.

I give permission for my details to be shared with the NHS Track and Trace system (issued through Coacha) so I may remain informed and inform others if they may be affected.

I state that I have read the Season Handbook in its entirety and agree to abide fully by the contents.
