Thank you for taking the time to complete the Dry Creek Elementary online kindergarten registration. Early online registration completion is helpful. We will take note of your requested track as soon as you complete this online form. It is not necessary to bring your required documentation into the office for us to record your track request. Please understand we will do everything we can to accommodate all requests.  

Sometime before school starts, registration paperwork must be signed in person in our office by a legal guardian (i.e., mother, father, court-appointed guardian).  

Below is a checklist of things you will need to bring with you to the school to complete registration sometime before school begins:
-Original Birth Certificate
-Picture ID

The documents listed below should be uploaded as soon as possible. They can be emailed (ascoresby@alpinedistrict.org) or faxed directly to the school office (801-766-8818).
-Proof of Residency (current utility bill, mortgage statement, lease agreement)
   *If living with others, contact the office for a form.
   *If living outside Dry Creek boundaries, you must submit an electronic Out of Area request through the following form: https://forms.alpineschools.org/out-of-area-request/.
-Immunizations can come directly from your physician's office.
-Guardianship/Custody Documents (if the child does not live with both parents)
