All clients are required to sign a liability waiver and that any personal injuries sustained are
the responsibility of the client and the client’s personal health insurance.

PARENT/GUARDIAN AGREEMENT I, the parent/guardian of the registered, minor participant agree to abide by the stated policies and pay training fees in full. Recognizing the possibility of physical injury associated with the fitness and training components activities, I hereby release, discharge and/or otherwise indemnify MIT and its affiliated organizations, partners and sponsors, their employees and associated personnel, including the owners of fields and facilities utilized for the service, against any claim by or on behalf of the registrant as a result of the registrant’s participation in the program and/or being transported to or from the same, which transportation I hereby authorize

Liability Regarding COVID-19, MIT is taking measures to mitigate any exposure to COVID-19 as stated on the website and governors guidelines, but because it is difficult to determine the origin of any exposure to COVID-19, all clients using the facilities at this time are doing so at their own risk and must hold MIT harmless if they do contract COVID-19, while using the facility.
