2024 OSDA Photography and Recording Waiver
By submitting this form, the individuals identified below agree to the following:
AUTHORIZATION AND WAIVER
I, either for myself or for the student below, understand that the Ohio Speech and Debate Association (“OSDA”) may take photographs, audio recordings, and audio-visual recordings of me or my student at the State Finals Tournament. I grant the OSDA all rights to use my or my student’s name, likeness, image, and voice in photographs, audio recordings, or audio-visual recordings taken at the State Finals Tournament in perpetuity and for any lawful purpose. Further, I waive any claims I or my student may have stemming from the creation, editing, or use of photographs, audio recordings, or audio-visual recordings taken at the State Finals Tournament, and I agree never to pursue any such claims against the OSDA, its Executive Officers, its Board Members, or any other agents of the OSDA.
If it were not for my agreement to execute this document—without modifications thereto—I acknowledge that I or my student would not be permitted to participate in the State Finals Tournament. I further acknowledge and agree that I understand all of the terms and conditions of this document.
