NON-Common App Transcript Request Form
THIS FORM IS FOR ANY COLLEGE THAT DOES NOT USE THE COMMON APP. Do not use this form for a college on the Common App!

1. We DO NOT send teacher's letters of recommendation, you MUST ask your teachers to send/upload to the college themselves. We suggest you give your teacher a stamped envelope with the college admissions address on the envelope. We only send the counselor's letter of recommendation.

2.  Standardized test score reports (SAT, ACT) must be sent directly to the colleges by the testing companies (College Board or ACT). The student should make these requests by going to their College Board or ACT account and completing the send scores process.

3. There is a MINIMUM of 10 day school period to process transcript and letter of recommendation requests. Do not wait until the last minute!

4. Please list each college individually in this form. If you have more than one NON-CommonApp college you must fill out a separate form for each college.
