2023 Finals Permit Application
1. Requests to use players from lower teams to play in the final rounds must be submitted via this form and will only be accepted by FNA from your Club’s nominated Permit Officer;

2. Forms for each Final Round must be submitted by 5pm on Wednesday before that final date;

3. Applications should be submitted as early as possible - at least a week prior if possible;

4. A separate application is required for each player;

6. Applications will be considered by the Permit & Protest Committee and Clubs advised of the outcome as early as possible;

7. Decisions will be made in accordance with FNA's Finals Permit Policy, available on the FNA website:

8. Late applications will only be considered in the case of an emergency, e.g. a late injury or illness, a sudden and significant change in personal circumstances, etc;

9. In cases of emergency, applications on the “Permit Application” form will be accepted up to the commencement of the game, noting that a permit player must have received approval prior to taking the court.

This form will ask you for the following information:

1. Details about the Match that requires a permit player (e.g. team and grade, ladder position, number of players in team);
2. Details about the player who will be unavailable, including reason for application and playing position of permit player required;
3. Details of the permit player (e.g. name and date of birth, their registered team and ladder position, representative team participation history);

It's recommended that you have this information available to you prior to starting the form.
