Administration of Medication Request (Administración de Solicitud de Medicación)

For the safety of all students at our school, these guidelines should be followed: 1. Administration of prescription and over-the-counter medicine (even for a short period of time) is discouraged. Parents should check with their physician regarding the need for medications to be administered during school hours. Medications prescribed for three times daily often can be given before school, after school, and at bedtime. If you have any questions about this procedure, please call the office. 2. All medications, both prescription and over-the-counter, must be accompanied by this form and brought to the office by an adult. (Once the medication and administration requirements have been added to the system, students may bring refills without needing a parent/guardian.) 3. All medications must be in the ORIGINAL CHILD PROOF CONTAINER. Prescription medications must be in the labeled prescription bottle. Pharmacists can give a duplicate labeled container with only the school dose. It is the responsibility of the parent/guardian to inform the school of any changes and update medication forms. Medications stored in envelopes, baggies/ziplocs, etc., will not be administered. 4. Medications must be picked up at the end of the year, or the school will dispose of them. ALL MEDICATIONS NEED TO BE ADMINISTERED ACCORDING TO DIRECTIONS ON LABEL

Por la seguridad de todos los estudiantes en nuestra escuela, se deben seguir estas pautas: 1. Se desaconseja la administración de medicamentos recetados y de venta libre (incluso por un corto período de tiempo). Los padres deben consultar con su médico sobre la necesidad de administrar medicamentos durante el horario escolar. Los medicamentos recetados tres veces al día a menudo se pueden administrar antes de la escuela, después de la escuela y antes de acostarse. Si tiene alguna pregunta sobre este procedimiento, llame a la oficina. 2. Todos los medicamentos, tanto recetados como de venta libre, deben ir acompañados de este formulario y deben ser llevados a la oficina por un adulto. Es responsabilidad del padre o tutor informar a la escuela sobre cualquier cambio y actualizar los formularios de medicamentos. No se administrarán medicamentos almacenados en sobres, bolsas/ ziplocs, etc. 4. Los medicamentos deben recogerse al final del año, o la escuela los eliminará. TODOS LOS MEDICAMENTOS DEBEN ADMINISTRARSE SEGÚN LAS INSTRUCCIONES EN LA ETIQUETA.
