This request form must be submitted at least one month prior to the date of the related event. Please look at each required event to see the deadlines.

Attendance at dress rehearsals and performances account for 25% of each student's attendance grade. Submitting this form does not guarantee the request will be approved. To be considered, this request must be submitted on or before the deadline that follows each event.

If you are requesting multiple dates, YOU MUST SUBMIT a separate form for each event.
