Aloha Families -- Weekly Covid Testing will now be two times per week -- Monday and Thursdays. If you signed up your child for weekly testing, please complete this survey so we know what your preference is. Do you want your child to be Covid tested 1x or 2x per week.

For parent who do not complete this form, your child will be covid tested only once per week. The staff will determine which day your child will be tested.
