Before filling in this form, please ensure that you are a member of the Panaga Club.

Music lessons at the Panaga Club are organised by the club volunteer-run Panaga Music Society.
Upon receipt of this form the music lesson registrar will contact you via e-mail to assign a teacher and lesson time. You will also be provided with further lesson information.

By completing this form you are providing authorisation for the club to deduct fees once the lessons commence.
