Liability Waiver

Event: Speed and Agility Clinic

Date: June 4th - 6th, 2024

I, [Participant's Full Name], hereby acknowledge and agree to the following terms and conditions set forth by The Rec, LLC in relation to my participation in the Speed and Agility Clinic:

1. Assumption of Risk: I am aware that participating in physical activities, including but not limited to speed and agility training, involves inherent risks and hazards. I understand and accept these risks, including the potential for injury or loss, and voluntarily choose to participate in the Speed and Agility Clinic.

2. Health and Fitness: I hereby confirm that I am physically fit and healthy to participate in the Speed and Agility Clinic. I am not aware of any medical condition, injury, or impairment that would prevent me from safely engaging in the activities involved. I understand the importance of consulting with a healthcare professional before participating if I have any concerns regarding my health or fitness.

3. Release of Liability: In consideration of being allowed to participate in the Speed and Agility Clinic, I hereby release, discharge, and hold harmless The Rec, LLC, its directors, employees, agents, and affiliates from any and all claims, liabilities, damages, actions, or causes of action arising out of or in connection with my participation in the clinic. This includes, but is not limited to, any injuries, accidents, illnesses, or property damage that may occur during or as a result of my participation.

4. Indemnification: I agree to indemnify and hold harmless The Rec, LLC, its directors, employees, agents, and affiliates from any and all claims, demands, damages, liabilities, and expenses, including attorney fees, arising from or related to my participation in the Speed and Agility Clinic.

5. Photography and Media: I grant The Rec, LLC the permission to capture and use photographs, videos, or other media of me during the Speed and Agility Clinic for promotional and marketing purposes, both online and offline. I understand that my personal information will be handled in accordance with applicable privacy laws and regulations.

6. Governing Law: This liability waiver shall be governed by and construed in accordance with the laws of [State], without regard to its conflict of laws principles.
