CUNY Policy only allows people fully vaccinated against Covid19 or people who have documentation of a negative PCR test from an accredited lab taken within the past 7 days to access campuses. Once you complete this form you will receive an email from Cleared4 requesting you to upload proof of vaccination. Please fill it out as soon as possible, so you will be allowed on campus.
For this reason, it is necessary for EVERYONE who plans to attend the event, including family members, to submit this form and to submit their proof of vaccination. We also ask that you wear a mask when in the dance studio for the safety of everyone. Thank you for your patience with this process. We are very excited to dance with you.
