CUEMS Course Request Form
Cornell University EMS is pleased to provide a variety of training and educational programs to the Cornell community free-of-charge, in an effort to prepare bystanders to act in the event of a medical emergency. CUEMS offers regularly scheduled courses throughout the academic year taught by some of our own members who are trained Basic Life Support Instructors, certified by the American Safety & Health Institute (ASHI).  

ASHI Certification and training courses offered:
 - ASHI Adult/Child/Infant Cardiopulmonary Resuscitation (CPR) with Automated External Defibrillator (AED) 
 - ASHI Basic First Aid
 - Combo CPR/AED and Basic First Aid

Non-certification training courses offered (email cuemscomed@cornell.edu if interested):
 - Alcohol Awareness and Safety (educational presentation, only for group requests)
 - "Stop The Bleed" Training

A weekly Adult CPR/AED Certification course is offered every weekend during the academic year from 10 AM - 1 PM. A Basic First Aid Course is offered every other weekend of the academic year from 1 PM-2:30 PM. If you are a Cornell affiliate and CPR/AED or Basic First Aid is the only certification course you currently seek, you may choose to bypass this form and do the following:
 - Log onto CULearn (www.culearn.cornell.edu) using your NetID.
 - Select "Find a Course." In the search bar, type "EHS 5350 AED/CPR", and/or "EHS 5360 First Aid."
 - Search for a scheduled class on a date of your choosing, and select "Enroll."

What is the expected number of people in the group? Please include yourself, if applicable.
IMPORTANT: We can only certify 10 people at a time. It is totally okay if your group exceeds 10 people, but please be aware that this means we will not be able to certify the entire group at one time. We can do multiple days/times in order to certify everyone. If your group exceeds 10 people, please be sure to list many possible course dates in the next question.
Preferred course dates/times
IMPORTANT: Please aim for preferred dates that are AT LEAST 2 WEEKS from today to allow our instructors an appropriate amount of time to plan ahead, and please offer multiple options, if possible. However, feel free to choose dates as far in advance as you would like.
I confirm that if I am requesting a class within two weeks or less of the date of form submission, I will email the CUEMS Community Education Officer (cuemscomed@cornell.edu) to inform them of this. *
I acknowledge that most, if not all, of our classes are taught by student volunteers who are largely unavailable during holidays, university breaks (including summer), finals weeks and large university events.
