LADI State Tournament 2022 Registration
Team Managers, please complete and submit a copy of this form for each of your school or organization's teams that plan to participate in the Louisiana State Destination Imagination Tournament on March 19, 2022.

The state tournament registration deadline is January 14, 2022.

The State Tournament registration fee is $50 per team, paid by check or money order made payable to Louisiana Destination Imagination.

You may include the cost of your T-shirts in the same check or money order with your tournament fee or write separate checks.

Payment for your team's tournament fee and T shirts (payable to Louisiana Destination Imagination) should be mailed to:

Make sure that you write your team's membership number on the check or money order.

The membership number begins with 118- and you received a separate number for each of your teams when you registered with Destination Imagination, Inc. through Shop DI.

T-SHIRT ORDERS:  The second section of this form will require you to submit your state tournament T-shirt order forms, so please only fill out the first section if you have T-shirt sizes and are prepared to submit your T-shirt orders in the 2nd section as well. Feel free to order T-shirts for your team's supports in addition to yourself and your team members.
