100 Women Who Care Mississauga - Member Registration (Individual) & Commitment Form 2021
Commitment: I agree that the information I provide below is accurate and true. I am pledging to participate in 100 Women Who Care Mississauga, and I am making a personal commitment to contribute $200 this calendar year (2021, $100 this spring, and $100 this fall) to local nonprofit organizations serving the city of Mississauga. Starting in 2022, there will be 3 Giving Circle meetings, Winter, Spring, Fall, 3x $100 = $300).

I agree to donate to the nonprofit organization selected by the group’s plurality vote (meaning the charity who gets the most votes). If I am unable to attend a Giving Circle meeting, I will either send my cheque with another attending member to deliver on my behalf, mail it as requested after the meeting, or pay online, if that option is presented.

I also acknowledge that photographs and videos taken at events and meetings may include my image and may be used in promotional materials for 100 Women Who Care Mississauga.
