2022-2023 CARES REGISTRATION FORM
CARES provides morning and after school care for students that need to be dropped off to school before the school day begins and/or picked up after the school day dismisses. Morning CARE (also known as Before Care) begins at 7:00 a.m. After CARE begins at the time of the student's dismissal and runs to 5:30 p.m. each evening. Students may bring snacks for both Morning and After CARE. Students may also bring breakfast with them for Morning CARE. Please complete the following form to register your child(ren). There is a $25.00 Registration Fee (per family) that will be billed at the end of the first month through Parish Giving. Students can enroll in CARES at any point throughout the school year and CARES can be used in the event of an emergency. All fees and costs still apply in these cases.

Please List those who are authorized to pick your child up from CARES on a normal basis. Saint Pio Regional Catholic School will not release your child to anyone who does not appear on this list or is not a designated Emergency Contact that is listed above.   *
A custody agreement is in place for my child(ren) and a current copy of this custody agreement is on file in Saint Pio Regional Catholic School's Main Office. *
