This form is for quarterly report submission for current trainers certified by the Office of the State Superintendent of Education (OSSE) Trainer Approval Program (TAP). Please complete this form to submit your quarterly reports. More information about the required quarterly reports may be found on pages 35-36 of the TAP Manual (June 2020).
