Select ONLY ONE answer for each section of questions.  For example, there are 5 questions labeled "Rank your attendance for the week."  Only mark the ONE answer for those five questions that matches your attendance for the week, and so on.  If you accidentally click on more than one answer in a section, you may select the "other" answer to discount the unitentionally selected response.
