2023-2024 SNPhA Membership Form
We are excited you have decided to join our SNPhAm! Please fill out the form below in its entirety. Completion of the form and payment of dues is required to become an official SNPhA member for the 2023-24 academic year.

The 2023-24 Membership Drive is open until end of day November 5th, 2023. Early deadline is October 15th, and completion by this early date guarantees a free SNPhA gift!

Dues are to be paid to our Treasurer, Krishna Patel, either by Venmo (@Krishna-Patel-122), cash, or other arrangements made with Emily Gaidos.

All you need to do is submit this form and pay your dues in order to become a SNPhA member and obtain a national SNPhA registration and website log-in. We will register for you, so please do not attempt to register yourself on the website. If you believe you have already registered this year and paid for a national SNPhA membership on their website, please contact Emily Gaidos.2@osu.edu before submitting any dues to us.

All members: $70 (this includes National SNPhA dues)

P3s: Can choose to pay dues for this year, or you can pay $105 for a combined P3/P4 year membership (a savings of $35)

You will still be counted as having met the early deadline even though we cannot collect your dues until the next GBM (10/24/2023), which falls after the early deadline has already passed.
