Roy High Summer Soccer Camp 2019
Roy High School girls soccer program will be hosting a Summer Soccer Camp June 4-6th at Sandridge Park for boys and girls in our community. Elementary students will be in the morning from 8-10 am.  Junior High students will be training from 10:30-12:30 pm with girls & boys soccer coaches Alyssa Foutz and  Brock Charlesworth. For those attending the RHS Basketball camp, we have added a summer soccer camp session from 4-6 pm.  Our soccer camp will emphasize skill development, game play and most of all FUN! Registration includes a camp t-shirt and a snack!
-Pre-register by May 17th and pay $30!
-Register by May 31st and pay $35!
-Sign up on the day of June 4th and pay $40!
* Payments can be made online for WSD students OR in person with the bookkeeper at Roy High School.
**WSD Students Online Payment Step By Step Instructions: Login to the student portal at myweber.wsd.net. Click Pay Fees then select your student. Click Items At All Schools, then High School, then Roy High. Click the Optional tab and you will see the RHS Summer Soccer Camp.

I will be attending the RHS Basketball Camp and therefore will be attending the 4-6 pm session. *
As the parent/ guardian of the applicant, I hereby grant permission to participate in the above camp at Roy High School and represent that they are physically able to participate in camp activities. In consideration of the applicants being allowed to participate in the camp, I hereby release the camp, Roy High School, Weber School District, its employees, officers and Regents from all claims resulting from illness, injuries or other damage which may be sustained by the child during attendance at the camp. I furthermore agree and promise that we will not hold Roy High School or any of the above parties responsible in this respect. This waiver of liability expressly includes transportation to, from or in connection with such camp. In the event of illness or injury, we hereby authorize the staff members of the camp to obtain assistance from doctors, nurses or athletic trainers for medical, surgical or any other appropriate treatment for the above mentioned child. Furthermore, I grant permission and consent for the attending physician to provide any medical or surgical treatment which, in the physician’s professional opinion, is deemed and necessary. If medical/ surgical care is obtained, we will not hold the camp or Roy High School, its employees or Regents responsible for liable for the judgment of and/or treatment by the physician. I understand that the camp director, Roy High School and the athletics department cannot assume responsibility for medical, dental or other health expenses incurred as a result of my child’s participation at the camp. I am the parent or legal guardian of the Participant. I have read this document, and by clicking "I agree" I am agreeing to the terms above. *
