2024-2025 Bus Transportation Information
Please include on this form EVERY STUDENT in your household who attends/will attend SMS and will be riding a bus at anytime during the school year. there is a place at the bottom of the form to include additional riders.

Students who will not need bus transportation(walkers, car riders) do not need to complete the form. As a reminder all students here via choice/lottery/transfer are not eligible for bus transportation.
