PHOTO RELEASE: I give Jade Park Tennis permission for myself, my child’s photograph(s) to appear on their Website, brochure, social media and/or newsletter with or without the name.

CANCELLATION POLICY: If you do not show up for your scheduled class or cancel with less than 24-hour notice, you will be charged full class fee except that you notify your urgent health issues or emergency before class start time. If weather cancellation is needed you will receive a group text at least 30 min prior to your lesson time unless weather changes unexpectedly. You are welcome to contact your coach for any question regarding weather cancelation.

Waiver: I hereby grant my permission, or as the parent/legal guardian for the above-named child, to attend tennis activities at Buchholz Highschool and I further authorize Jade Park tennis staff to act for me in their best judgment in any emergency requiring medical attention in the event I may not be reached. I hereby waive Jade Park tennis, their staff and coaches for all liability for any injuries incurred while involving Tennis Programs with Jade Park Tennis. By signing below, it is acknowledged that all participants and parents must be advised that neither Jade Park, coaches, and volunteers are responsible in any manner for any injury that relates to or may result from activity in this program or any related program in any way. Any damage that may occur because of the action of any program participant, or affiliate person, will be the responsibility of that person (or parent/guardian). all its affiliates assume no responsibility, in any way, for the actions that may involve the above, or anything that may be detrimental to the above, either financial or otherwise.

Camp fee needs to be paid in advance to reserve your spot. We reserve the right to cancel or shorten camp hour with fewer than 8 students.
Prorated refunds will be made or credit will be given when we cancel.
No credit if you miss for any reason.
