Puget Sound Revels (PSR) COVID-19 Consent and Protocol
Protocols for adult participants in events/performances/rehearsals

I understand that the COVID-19 virus (and variants) is highly contagious and can cause known, unknown, foreseen, and unforeseeable risks; that it poses health risks to those who contract it and to those who come into contact with individuals who have contracted it; and that it poses a higher risk to some individuals - those who are immunocompromised, have chronic medical conditions, are pregnant, and older adults. I understand it may cause illness and symptoms including fever, cough, shortness of breath, mild to severe respiratory illness, and death. I understand that, while PSR is mitigating risk by following state health guidelines and protocols and require vaccinations for those of age, PSR cannot eliminate the virus from any public environment.
PSR rehearsal and performance protocols are as follows:
1. Participants who are eligible for a vaccine will be vaccinated.
2. No performer, production/artistic staff member should attend an in-person PSR event if that person has any of the following possible signs of COVID-19:
• Fever (100.4 or higher) or chills • Cough
• Shortness of breath or difficulty breathing • Fatigue
• Muscle or body aches • Headache
• New loss of taste or smell

3. Masks and social distancing will be required for all students, volunteers, and staff members in conformance with applicable Washington State and Pierce County guidelines.
4. Participants will bring all required materials to rehearsal and will not share personal items with other members (ie water bottles, phones, pencils, music, etc.)
5. This protocol is subject to change in conformance with changes in laws, orders, or guidelines from government authorities or health agencies.

I am vaccinated and agree that if I show any sign of illness (or test positive without signs of illness), I will not attend any PSR event, rehearsal, performance. I agree that if I or an individual I have been in close contact with is confirmed to have COVID-19, I will refrain from attending an in-person PSR event as outlined in the PSR COVID-19 Protocol.

I waive, release, and will hold harmless PSR, its agents, staff, directors, and volunteers of all claims that may arise out of or in connection with and/or related in any way to COVID-19.
