It is important to us that all students, staff, post-docs, faculty, and visitors feel safe and respected in our classrooms and labs and that policies and structures promote lifelong learning for all.

This form is one option for sharing feedback with our departmental administrators about serious issues in our department. You may choose to share the information anonymously or provide your contact information so that the administrators can follow up.  

As a first step, we generally recommend that you speak with the instructor of a course or your research mentor to attempt to rectify a situation. Below are some scenarios with the suggested action.
