This form is for people playing in a Premier grade (including Youth playing up) applying for APs at Western. This year we're offering 3 options for these APs to suit a variety of situations. If you plan to pay via AP this year, you will need to fill out the following form and organise your payments no later than the 10th of April, 2020. Any alterations to the AP resulting in a later than agreed finalisation date will incur the $50 late fee.
