2024 Grundy Field Day Shirt Order Form

The Grundy PTA gifts each K-4th grade student and Grundy staffers our special 2024 Field Day shirts. In order to receive them, please fill out the below form by May 3rd. 1 per student/staffer

(please fill out a new form for each Grundy student). ALL SIZING IS FINAL. There are no exchanges for the wrong size we recommend ordering up since the students can wear the shirts during future spirit days. Extras to purchase will be available at Grundy Store once the designed is finalized.
