For any of the questions below, if you cannot answer, please leave blank.  If you have found numerous issues or bugs, complete another bug report for each issue.  Please also try to provide at least one way to contact you (either through Discord, Telegram or Twitter) as we may need more information such as screenshots of your issue.  We will also be basing our bug bounty rewards on these submissions and any reward given is subject to our discretion.
