Oceana recognizes its responsibilities under the Republic Act
No. 10173 (Act), also known as the Data Privacy Act of 2012, with respect to
the data they collect, record, organize, update, use, consolidate or destruct
from the participants/attendees/organizers. The personal data obtained from
this portal is entered and stored within Oceana’s authorized information and
communications system and will only be accessed by Oceana authorized personnel.
Oceana has instituted appropriate organizational, technical and physical
security measures to ensure the protection of the
participants/attendees/organizers’ personal data.
Oceana shall not disclose the participants/attendees/organizers’ personal
information without their consent.
PARTICIPANT'S CONSENT
I have read the Data Privacy Statement and express my consent for Oceana to
collect, record, organize, update or modify, retrieve, consult, use,
consolidate, block, erase or destruct my personal data as part of my
information.
I hereby
affirm my right to be informed, object to processing, access and rectify,
suspend or withdraw my personal data, and be indemnified in case of damages
pursuant to the provisions of the Republic Act No. 10173 of the Philippines,
Data Privacy Act of 2012 and its corresponding Implementing Rules and
Regulations.
TALENT RELEASE WAIVER
I agree to have my images and statements taken by photo or video by Oceana Philippines International (“Oceana”) and its representatives without compensation. I hereby give my consent to the processing of any relevant personal information which is necessary, related to the fulfillment of this subject form, and for purposes of the legitimate interests of Oceana to whom the data is disclosed.
I give Oceana permission to use my name, photograph, and likeness (“Likeness”) for the above purposes. I hereby waive any right of inspection or approval of my Likeness or the uses to which such Likeness may be put. I release Oceana, its staff, and directors from any liability for use of my Likeness in emails, Public Service Announcements, newsletters, the Internet, and any and all other media, promotions, or publications.
Oceana owns all rights in perpetuity to the material (i.e., images in all formats and media) that result from my participation in these activities. That is, if my participation is determined to be a “work for hire,” I agree that such copyrights belong to Oceana, and if not, I convey to Oceana such copyrights in exchange for participation in these activities and other consideration.
